Camp Policies
Medical Release
I fully understand that Naptown Jazz Kids, Inc’s (NJK) staff are not physicians or medical practitioners of any kind. With that in mind, I hereby authorize the NJK staff to render first aid to my musician in the event of any injury or illness, and if deemed necessary by NJK staff to call a doctor and to seek medical help, including transportation by NJK staff or representatives, whether paid or volunteer, to any health care facility or hospital, or the calling of an ambulance for my musician should the NJK staff deem this to be necessary.
Social Media Release
I hereby give permission to Naptown Jazz Kids (NJK) to photograph and/or videotape my musician. The sole purpose of these photos and/or videos is for publication, advertisement, and exhibition of services by NJK.
Behavior Expectations
I understand and agree that my child must abide by all rules, regulations, expectations and standards of conduct applicable to participation in NJK. I further understand and agree that NJK reserves the right to suspend, limit or terminate my child’s participation in any activity, or in the NJK if, in the sole discretion of NJK staff, my child’s conduct or actions do not conform to said rules, regulations, expectations, and standards of conduct or are otherwise dangerous, destructive, or disruptive.
Refunds
Refunds are available if registration is cancelled at least 6 weeks prior to the first day of the summer program, though a $100 service fee will be applied.
